If you need or desire assistance from a private duty companion, sitter, or third party provider, You and the provider must abide by the Community Policy and Procedure set forth for third party providers. You agree that before service begins, the provider must meet with the Community Director or Resident Services Director for appropriate orientation and policy requirements.  Should the private duty companion/sitter or third party provider violate Community Policy, Eastridge Assisted Living reserves the right to expel or remove the third party provider from the Community.  Your private duty companion, sitter, or third party provider is your sole responsibility and is not representative or agent of Eastridge Assisted Living.  You are solely responsible for selecting, screening, and directing such person, and it is agreed that Eastridge Assisted Living will have no liability or responsibility for the actions of Your third party provider.  You agree to indemnify and hold Eastridge Assisted Living harmless from any liability for claims for personal injury or property damage caused by the conduct of Your private duty companion, sitter, or third party provider.
